If you wish to address the Commission on any matter, please fill out one of the forms located at the speaker's podium so that your remarks within the recording of the meeting can be attributed to you. Completion of this form is not mandatory, but accurately provides your information for the record. CONSENT CALENDAR: All matters listed under the Consent Calendar are considered routine and will be enacted by one motion. There will be no separate discussion of these items prior to the time of the voting on the motion unless members of the Commission, staff, or public request specific items to be discussed and /or removed from the Consent Calendar for separate action. Written materials relating to agenda Items for an open session regular meeting of the Planning Commission that are distributed to a majority of the Planning Commission less than 72 hours prior to the meeting are available for public inspection at the following locations: At the Community Development Department — Planning counter - during regular business hours and following business hours (after 5:30 p.m.) in the Council Chamber on the day of the meeting. APPROVAL OF MINUTES — APRIL 8, 2014, PLANNING COMMISSION MEETING. RECOMMENDATION: That the Planning Commission approve the minutes of the April 8, 2014 meeting as provided. CONDITIONAL USE PERMIT 2013 -13 & DESIGN REVIEW 2013 -11 A request to construct and operate an unmanned wireless telecommunications facility consisting of a fifty (50) foot tall mono - eucalyptus faux tree with twelve (12) panel antennas and associated equipment. ENVIRONMENTAL: This project is categorically exempt pursuant to Section 15303 (Class 3) of the California Environmental Quality Act (CEQA). RECOMMENDATION: That the Planning Commission adopt Resolution No. 4256 approving Conditional Use Permit (CUP) 2013 -13 and Design Review 2013 -11, a request to construct and operate an unmanned wireless telecommunications facility consisting of a fifty (50) foot tall mono - eucalyptus faux tree, twelve (12) panel antennas and associated equipment to be located behind the existing building at 14450 Chambers Road.
