Claimant's name is MATTHEW BROWNDORF, and he resides at 2. This claim arose out of an incident which occurred on October 25, 1991, in the City of'Tustin, near the intersection of Williams Avenue and McFadden, at approximately 6:30 p.m. At or about that time, a police dog under the direction of a uniformed 1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 i 24 25 26 27 28 police officer employed by the City of Tustin, attacked claimant, causing him serious bodily harm and caused him to seek medical and psychiatric treatment. 4. Claimant is uncertain of the exact amount of his claim for his injuries, but believes the damages to be in excess of $50,000.00. 5. Claimant is uncertain of the names of the police offi- cers who were controlling the police dog which attacked claimant, but believes that the City of Tustin Police Department has that information in its records of the incident.
