Avenue, Tustin, California. Circumstan..ces of Occurrence: On' January. 26, 1988, at the above Claimant was on the walking --1-- mentioned place at or about sidewalk near 13362 Newport 3 6 7 8 9 10 11 12 ~.3 15 16 .19 20 21 23 24 25 26 27 Avenue. Claimant did not see a defect in the sidewalk and tripped and fell to the ground. 6. General description of injury, damages or loss incurred: Claimant has been injured to her health, strength and activity, sustaining injuries to her body and nervous system. These injuries have caused, and will continue to cause Claimant mental and physical nervous pain and suffering. Claimant is informed and believes and therefore alleges that the injuries will result in some permanent disability to Claimant. 7. Further, said injury has caused Claimant not to be able to attend to her usual occupation. 8. The names of any public employees known to cause injury are unknown to Claimant at this time. 9. Damages claimed as of the date of this Notice are $50,000.00.
