ISPACE BELOW FOR FILING STAMP ONLY) CLAIM FOR PERSONAL INJURY (GOVERNMENT CODE §910) TO: RESPONDENT, CITY OF TUSTIN: YOU ARE HEREBY NOTIFIED that JON MOORE, who resides at claims damages by way of this claim from the CITY OF TUSTIN, in the amount computed as of the date of the presentation of this claim of $150,000.00. This claim is based upon personal injuries sustained by the claimant, JON MOORE, on February 9, 1990, while walking on the sidewalk adjacent to the area of the southeast corner on Newport Avenue at the intersection with Main Street, Jn the City of Tustin, County of Orange, State of California. At approximately 8:00 p.m., at the above mentioned location, claimant received severe and substantial injuries due to the - 1 - 1 dangerous condition of the sidewalk and sidewalk surface. The 2 dangerous condition of the sidewalk and sidewalk surface included 3 what was not limited to the following: 4 The sidewalk had dangerous posts of a metal chain link fence 5 which protruding and extending outward into an area designated for 6 pedestrian use. The dangerous said objects were unmarked and 7 extended into the path of persons walking on said sidewalk thereby 8 rendering the sidewalk dangerous to pedestrians. 9 As a result of the dangerous condition as herein above 10 described, claimant, JON MOORE, was caused to trip and fall as he 11 walked on the sidewalk adjacent to the area of the southeast 12 corner on Newport Avenue at the intersection with Main Street, in = o - oW8 13 the City of Tustin, County of Orange, State of California. As a Z- a o 14 result of the fall, claimant, JON MOORE, sustained severe and 11 Z U0 LL 0 X " 15 substantial injuries to his person. S=x�" 16 At the present time, claimant is informed and believes that F vn� 17 respondents, CITY OF TUSTIN, control and/or maintain the sidewalk 18 and area referred to above, and/or employ those public employees 19 or contractors who had the responsibility for the aforementioned 20 sidewalk and area. 21 The names of public employee and/or contractor causing 22 claimants injuries under the described circumstances are unknown 23 and unascertainable at this time. 24 The injuries sustained by claimant, as far as known as of the 25 date, of presentation of this claim, consist of, among other 26 things, head injury,.hip injury, neck and back injury and dental 27 injury. 28 The amount claimed as of the date of presentation of this _ 2 _ 1 claim is computed as follows: 2 Damages incurred to date: 3 Expenses for medical and hospital care......
