The name and post office address of JAY PHOTOGLOU is 2. All notices relative to this Claim should be sent to my attorney, RONALD D. MACGREGOR, 321 West 17th Street, Santa Ana, California 92706. 3. On or about April 22, 1990, at or near the I-5 freeway, northbound, its interchange with the SR -55 freeway, northbound, City of Tustin, County of Orange, State of California, JAY PHOTOGLOU was a passenger in a motor vehicle which was involved in a single vehicle collision which proximately resulted in personal injury. 4. Claimant claims on information and belief, that the location where -the accident occurred consisted of a dangerous condition of public property which proximately contributed to the accident and injuries of JAY PHOTOGLOU. 5. Claimant suffered abrasions to his left arm and side, injury to his legs, neck, back and other injuries as yet unknown. 6. I am over eighteen years and not a party to the within action. My business address is 321 West 17th Street, Santa Ana, California 92706. On July 25, 1990 I served the foregoing CLAIM on all parties in this action by placing a true copy thereof, enclosed in a sealed envelope with postage thereon fully prepaid and addressed as follows: CLERK OF THE CITY OF TUSTIN 300 CENTENNIAL WAY TUSTIN, CA 92680 I am "readily familiar" with the firmis practice of collection and processing correspondence for mailing. Under that practice it would be deposited with the U.S. Postal Service on that same day as Santa Ana, California in the ordinary course of business. I am aware that on motion of the party served service is presumed invalid if postal cancellation date or postage meter date is more than one day after date of deposit for mailing in affidavit.
