. Manner of Injury: Claimant was a passenger in a vehicle
traveling eastbound on McFadden Avenue in the #1 lane when
another vehicle traveling noraD, bound on Walnut 'failed to yield
at a stop sign and collided with Claimant's vehicle causing said
vehicle to lose control and strike a brick wall. Said collision
was the proximate result of the negligent, construction, design
and maintenance of this particular section of roadway,
specifically, but not limited to, the lack of.sufficient
clearance and width of roadway lanes by design, insufficient and
improper placement of barriers and/or curbing so as to prevent
vehicles from losing contrQ% after hitting, and failure to have
collapsible barriers, all of which contributed to Claimant's
injuries.
5. Nature and exigent of injuries: Claimant sustained a
head injury wi~h sutures, soft tissue injuries to the ches~
area, and a puncture wound in the right foot.
6. Amount of damages claimed to da=e: $!,000,000.00

injuries, medical expenses and property damage, the amounts cf
ยท ~hich are presently unknown to Claimant. Claimant has ncr
received, any statements from anyone.
