CLAIM FOR DAMAGES
1. Name of Claimant: Catherine Bankston.

2. Date of Injury: April 22, 1986.
Place of Injury: State Route 55, south of the north
edge of Fairhaven Avenue over-crossing, City of Tustin, County
of Orange, State of California.
Manner of Injury: Claimant was a passenger in a motor
vehicle traveling southbound On State Route 55 when it was rear-
ended by another vehicle. Upon impact, Claimant's vehicle was
knocked to the shoulder of the highway, hit a shoulder berm, and
~verturned. Claiman%'s injuries were the proximate result of
the negligent construction, design, and maintenance of this
particular section of the highway; specifically, but not limited
to, lack of barriers, dangerous and inadequata off-road recovery .

3. Nature and extent of injuries: Claimant sustained
the following injuries: post traumatic head-syndrome,
musculoligamentous cervical strain, rule out cervical radicul-
pathy, musculoligamentous lumbosacral strain, contusion and
hematoma left hand, contusion and strain right knee.
4. Amount of damages claimed: $5,000,000.00 sum includes
damages' for present injuries and prospective injuries, medical
expenses and property damage, the amounts of which are presently
unknown to Claimant. Claimant has not received any statements
from anyone.
