CONWAY, Claimant, CITY OF TUSTIN, Governmental Entity. TO: CITY OF TUSTIN : Claimant, CAROLYN W. CONWAY, hereby makes claim against the City of Tustin for the sum of $582,000.00 and makes the following statement in support of the claim. 1. Claimant's post office address is c/o LAW OFFICE OF JOHN A. ROSENBAUM, 1801 E. Parkcourt Place, Suite E-102, Santa Ana, California 92701, and notices concerning the above claim should be sent to her at that address. 2. The date and place of the occurrence giving rise to the claim are December 15, 1984 au the intersection of First Street and North Tustin Ave. in the City of Tustin, California. 3. The circumstances giving rise to the claim are as follows: 1 2 3 4 5 6 7 9 10 11 12 13 14 15 16 17 18 19 2O 21 23 24 26 27 28 At the above time and place, Claimant was a pedestrian, walking in a cross walk in a legal manner from the northwest corner of N. Tustin Ave. at it's intersection with First Street to the northeast corner of N. Tustia Ave. at it's intersection with First Street. At said time and place the Cit~ of Tustin was negligent and otherwise acted tortiously with respect to the ownership, design, maintenance, supervision and repair of the intersection and its lighting and signal devices so as to cause and/or contribute to claimant being run over by a vehicle turning north (left) from eastbound First Street onto northbound Tustin Avenue. 4. Claimant's injuries are as follows: compound fractures at various locations in both legs, broken left ankle, broken left heel, broken right knee, probable torn meniscus both knees requiring knee replacment in the future, probable herniated disc in lumbar spine at L4-5, LS-S1 along with generalized bruises and abrasions. 5. Claimant's claim as of the date of this claim is in the amount of $582,000.00. 6.
