1. The Post Office address, of KENNETH Si GOH is
as follows: Kenneth Gob c/o Albert Gob, M.D., P.O. Box 244,
Tustin, California 92680.
2. The Post Office address to which Claimant,
KENNETH S. GOH, d~sires notice of this claim to be sent is as
follows: The Law Offices of Edward B. Djang, 910 East
Chapman Avenue, Orange, California 92666.
3. On April 23, 1984, Claimant, a sixteen (16)
year old student at Foothill High School, was proceeding
eastbound on Seventeenth Street approaching Holt Avenue in
the City of Tustin, when a vehicle driven by AUDREY JHAN TAYLOR

turned left from Seventeenth onto Holt Avenue, and the
Claimant was unable to stop his motorcycle in time and struck
the automobile near the front passenger door.
4. As a result of this collision, Claimant
sustained a broken tibia and ankle in addition to abrasions
and other assorted injuries. The intersection of Seventeenth
Street and Holt Avenue is negligently constructed and/or
maintained in that the drivers approaching this intersection
have insufficient time to view traffic coming from the
opposite direction and which negligent condition has lead
proximately to the injuries sustained by Claimant.
5. So far as it is known to KENNETH S. GOH at the
date of filing this claim, he has incurred damages in the amount
of approximately Five Thousand Dollars ($5,000) for the
injuries specified above, not including his claim for
general damages.
6. Claimant does not know the n~es of any public
employers or employees involved in this dangerous condition
and name them herein as DOES 1 through 5, inclusive.
7. At. the time o~ presentation of this claim,
KENNETH S. GOH claims damages in the amount of approximately
One Hundred Thousand Dollars ($100,000) or more based on the
medical expenses, lost wages, general damages and other damages
incurred as a result of the collision. It is presently
unknown whether Claimant will have permanent disability and,
if so, the extent of such disability so the amount of the
