of the Government Code, the undersigned herewith presents a
claim for damages to the CITY OF TUSTIN
The undersigned states as follows:
1. Name of Claimant: ROD L. CARPENTER

7/31/81
5. The place and other circumstances of the occurrences
or transaction .giving rights to the claim: On 7/31/81, at
approximately 11:30 a.m., at McFadden and Ritchey in the City
of ~TuStin ~, a 1979 Toyota automobile collided with Rod
Carpenter who was on a motorcycle, due to an obstruction of
view caused by a large mound of dirt that had been negligently
allowed to accumulate in the intersection due to street
construction.
6. Description of the indebtedness, obligation, injury,
damage or loss incurred: Property damage t° motorcycle, and
claimant sustained multiple abrasions, sprained arm, back
injuries and other injuries not known at this time.
7. The names of the public employees causing the injury,
damage, or loss, if known: unknown at ~his time.
8. The amount claimed together with the basis of
computation: Property damage - $2,000.00, personal injury
damage in excess of $15,000.00, exact amount unknown as he
1s still treating.
