VERIFIED CLAIM FOR DAMAGES
Pursuant to the provisions of Division 3.6 of Title I of the
Government Code, the undersigned herewith presents a claim for damages
to the CITY OF TUSTIN, a Municipal Corporation:
The undersigned states as follows:
1. Name of Claimant: EDWARD MATHEW THOMSON
2. Post office Address of Claimant: c/o Orange County Legal Clinic,
505 N. Tustin Avenue, Suite 228, Santa Aha, California 92705
3. Post Office Address to which Claimant desires notices to be
sent: Same as above.
4. The date of the occurrence or transaction: Sept. 19, 1978
5. The place and other circumstances of the occurrences or trans-
action giving rights to the claim: Vehicular accident between 1978 Ford
driven by Ronald D. Fleig and 1975 Honda motorcycle driven by claimant.
See Exhibit "A" (copy of Tustin Police Dept. traffic collision report
No 38-7451) attached to and incorporated by reference herein for details
of accident. A city owned signal light was defective and cause of
accident.
6. Description of the indebtedness, obligation, injury, damage
or loss incurred: Broken left thumb, broken right wrist, lacerations
to right hand and right knee and other injuries. Amount of medical
treatment and other losses have not be ascertained at this time.
7. The names of the public employees causing the injury, damage,
or loss, if known: Unknown at this time
8. The amount claimed together with the basis of computation:
Medical and other losses to be ascertained; pain and suffering in
the amount of $10,000.00.
