HUSTON, CITY MANAGER PUBLIC WORKS DEPARTMENT / WATER DIVISION CONSULTANT SERVICES AGREEMENT FOR THE NEWPORT AVENUE RESERVOIR IMPROVEMENT PROJECT (CIP NO. 6149) SUMMARY The Agreement with Harper & Associates Engineering, Inc. will provide the City with a topographic site and utility survey, reservoir cleaning, inspection and evaluation, and recommendatons for site grading and safety improvements. RECOMMENDATION It is recommended that the City Council approve the transfer of funds from the John Lyttle Reservoir Repair Project (CIP No. 6148) in the amount of $15,000 to the Newport Avenue Reservoir Improvement Project (CIP No. 6149); and Approve the Consultant Services Agreement with Harper & Associates Engineering, Inc., of Corona, California, in the amount of $27,109.75 and authorize the Mayor and City Clerk to execute the agreement on behalf of the City. FISCAL IMPACT A budget transfer will be required from the John Lyttle Reservoir Repair Project in the amount of $15,000 to the Newport Avenue Reservoir Improvement Project (CIP No. 6149). The John Lyttle Reservoir Repair Project (CIP 6148), which has an adjusted budget of $24,050, will be rescheduled for a future year. BACKGROUND The Newport Avenue Reservoir, constructed in 1926, is located on Newport Avenue south of Lemon Hill Drive (Attachment 1). The reservoir is a partially buried, reinforced concrete structure with a wood framed corrugated metal roof. The reservoir is 100 feet in diameter with a nominal storage capacity of 1.4 million gallons (mg). No as-built records of the 78 year old reservoir or other facilities that are located on the 0.41 acre parcel exist. The parcel includes the reservoir, an Orange County Flood Control District storm drain line, a Southern California Water Company (SCWC) transmission main, an East Orange County Water District (EOCWD) metering vault, and Tustin Water Service vaults, mains and other appurtenances. The site lies directly north and uphill of, a SCWC reservoir and booster pump station. Consultant Services Agreement for the Newport Avenue Reservoir Improvement Project January 5, 2004 Page 2 To address the lack of existing site and reservoir condition information, the Consultant will perform a topographic site survey, prepare a site utility and a site drainage plan, clean the interior of the reservoir and photograph/document the interior and exterior conditions, and evaluate the reservoir for OSHA compliance. Additionally, the Consultant will prepare a report documenting their evaluations, findings, and recommendations. Estimated construction costs for each item identified will be included in the report. The report and recommendations will be included in future capital improvement projects. Eight Requests for Proposals were solicited from a select list of consultants on September 5, '2003, and four consultants submitted Proposals on September 29, 2003. Based on the consultant's and sub consultant's qualifications, project understanding and approach, staff recommends that Harper & Associates Engineering, Inc. be selected to perform the activities required for a not-to-exceed fee of $27,109.75. Ragland Water Services Manager TDS:BAR:ccg:CSA Newport Ave. Reservoir Improvement.doc Attachments: Project Location Map Consultant Services Agreement CONSULTANT SERVICES AGREEMENT THIS AGREEMENT FOR CONSULTANT SERVICES, is made and entered into this __ day of , 20_, by and between the CITY OF TUSTIN, a municipal corporation, hereinafter referred to as "City", and HARPER & ASSOCIATES ENGINEERING, INC., hereinafter referred to as "Consultant". RECITALS WHEREAS, City requires the services of a consultant to furnish, the necessary engineering services for the Newport Ave. Reservoir CleaninR, inspection, and As-Built Site Plan hereinafter referred to as "Project"; and WHEREAS, City has prepared a Request for Proposal dated September 5~ 2003 and an Addendum to Newport Ave. Reservoir RFP dated September 23, 2003 a copy of which is attached hereto, marked as Exhibit "A" and is by this reference incorporated into_this Agreement; and WHEREAS, Consultant has submitted to City a proposal dated October 6, 2003, a copy of which is attached hereto marked as Exhibit "B" and is by this reference incorporated into this Agreement; and WHEREAS, Consultant is qualified to provide the necessary services for the Project and desires to provide said services; and WHEREAS, City desires to retain the services of Consultant for said Project. NOW, THEREFORE, for the consideration and upon the terms and conditions hereinafter set forth, the parties agree as follows: 174821.1 10/2003 AGREEMENT Section 1: Scope of Consultant's Services Consultant shall perform all work necessary to complete in a manner satisfactory to City, the services set forth in Exhibit "A". Section 2: Order of Precedence In the event of a conflict between or among any of the documents comprising this Agreement, the following order of precedence shall govern the provision in question: 1. This Agreement 2. City's Request for Proposal and Addendum to Newpbrt Ave. Reservoir RFP (Exhibit "A") 3. Consultant's Proposal (Exhibit "B") Section 3: Time for Completion The time for completion of the work to be performed by Consultant is an essential condition of this Agreement. Consultant shall prosecute regularly and diligently the work of this Agreement according to reasonable schedules established by the City for various items described and as outlined within Consultant's proposal. Consultant shall not be accountable for delays in the progress of its work caused by any condition beyond its control and without the fault or negligence of Consultant. Section 4: Compensation The compensation to be paid under this Agreement shall be as set forth in Exhibit "B", not to exceed a total cost of $27,109.75 174821.1 Consultant shall submit detailed invoices, based upon the actual work performed accompanied by backup documentation as requested by the City. 10/2003 2 Progress payments for work completed shall be paid by City as the work progresses, within thirty (30) days of the date of Consultant's invoice. Consultant shall provide City with a monthly itemization of all work performed, and the fees accrued thereon, in complete and sufficient detail to fully apprise City thereof Section 5: Independent Contractor Consultant's relationship to City in the performance of this Agreement is that of an independent contractor. Consultant's personnel performing services under this Agreement shall at all times be under Consultant's exclusive direction and control and shall be employees of Consultant and not employees of City. Consultant shall pay all wages, salaries and other amounts due its employees in connection with this Agreement and shall be responsible for all reports and obligations respecting them, such as social security, income tax withholding, unemployment compensation, workers' compensation and similar matters. Section 6: Indemnification Consultant agrees to indemnify, defend and hold City, its officers, agents, employees, successors and assigns harmless from any loss, damage, injury, sickness, death, or other claim made by any person and from all costs, expenses and charges including attorney's fees caused by or arising out of Consultant's, its officers', agents', subcontractors', or employees' negligent acts, negligent errors, or negligent omissions or willful misconduct, or conduct for which the law imposes strict liability on Consultant in the performance or failure to perform this Agreement. Section 7: Insurance 174821.1 Consultant shall maintain in full force and effect during the term of these Agreement policies of commercial general liability and automobile liability insurance (each of which shall include property damage and bodily injury) and each with limits of at least $1,000,000 combined single limit coverage per occurrence. 10/2003 Consultant shall maintain in full force and effect during the term of this Agreement a policy of professional liability insurance coverage with limits of at least $1,000,000 combined single limit coverage per claim or per occurrence. If Consultant provides claims made professional liability insurance, Consultant shall also agree in writing either (1) to purchase tail insurance in the amount required by this Agreement or to cover claims made within five (5) years of the completion of Consultant's service under this Agreement, or (2) to maintain professional liability insurance coverage with the same carrier in the amount required by this Agreement for at least five (5) years after completion of Consultant's services under this Agreement. Consultant shall also provide evidence to the City of the purchase of the required tail insurance or continuation of the professional liability policy by executing the attached Letter Agreement on Consultant's letterhead. Consultant shall carry and pay for such workers' compensation insurance as is required fully protect Consultant and its employees under California Worker's Compensation Insurance Law. The insurance company shall agree to waive all rights of subrogation against the City for losses paid under the policy, which losses arose from the work performed by the named insured. Do Other applicable insurance requirements are: (1) Name the City, its officials and employees as an additional insured on the commercial, general and automobile policies. (2) The insurance shall be issued by a company authorized by the Insurance Department of the State of California and rated A-, X (ten) or better, by the latest edition of Best's Key Rating Guide, except that the City will accept workers' compensation insurance rated B-, VII (seven) or better or from the State Compensation Fund. (3) The Insurance shall not be cancelled, except after thirty (30) days written prior notice to the City; and (4) The commercial general and automobile liability insurance shall each be primary as respects the City, and any other insurance maintained by the City shall be in excess of this insurance and not contribute to it. 17,4821.1 Upon execution of this Agreement, Consultant shall provide to City certificates of insurance and insurer endorsements evidencing the required insurance. Insurer endorsements (or a copy of the policy binder if applicable) shall be provided as evidence of meeting the requirements of Subsections (1)(3) and (4) of Section 7D above and the waiver of 10/2003 subrogation requirement in Section C above. If self-insured for worker's compensation, Consultant shall submit to City a copy of its certification of selfqnsurance issued by the Department of Industrial Relations. Section 8: Termination of A.qreement City and Consultant shall each have the right to terminate any or all of the services covered by this Agreement at any time for any reason by giving ten (10) business day written advance notice to the other party. Upon termination of this Agreement, Consultant shall be paid for services rendered by the effective date of the termination. Upon termination of this Agreement or completion of the Project, all documents relating to the Project shall become the sole property of City. Should City terminate this Agreement pursuant to subparagraph A. of this Section, Consultant shall, within ten (10) business days of receipt of notice of termination, provide City with all documents within Consultant's possession relating to this Agreement and the Project, including but not .limited to all completed documents and all drafts of uncompleted documents. ONTARIO AVE., STE 102-312 CORONA, CA 92881-8671 Section 10: Miscellaneous Provisions Consultant shall proceed immediately and diligently to perform the services provided for in this Agreement upon receipt of notice from City to proceed therewith. No part of this Agreement may be assigned by Consultant without the prior written approval of City. This Agreement shall extend to and be binding upon and inure to the benefit of heirs, executors, administrators, successors and assigns of the respective parties hereto. Consultant shall perform all services' required under this Agreement using that degree of care and skill ordinarily exercised under similar conditions in similar localities, and shall be responsible for all errors and omissions for services performed by Consultant under the terms of this Agreement. Consultant certifies that there shall be no discrimination against any employee who is employed in the work covered by this Agreement, or against any application far such employment, because of race, religion, color, sex or national origin including but not limited to, the following: employment, upgrading, demotion or transfer, recruitment, or recruitment advertising, lay-off or termination, rate of pay or other forms of compensation, and selection for training, including apprenticeship. This Agreement shall be interpreted in accordance with California Law. Ontario AVenue, Sutie 102-312 Corona, CA 92881-8671 City of Tustin 1472 Service Road Tustin, CA 92780 (714) 573~3375 FAX (714) 259-0074 Re: Request for Proposal: Newport Reservoir Repairs (ClP 6149) Dear Mrs. Harper: This letter represents a Request for Proposal for Consulting Engineering Services for the preparation of design plans, specifications, shop drawing reviews, and construction support services for the Newport Reservoir Repair (CIP 6149). The City of Tustin endeavors to retain a qualified experienced Consultant to perform the work required. INTRODUCTION The City of Tustin (City) is requesting proposals from interested firms to provide consulting services for the repair of the Newport Reservoir. This RFP describeS the proposal format, submittal requirements, preliminary scope of services and project schedule, the minimum information that must be included in the Proposal, and the selection process. Failure to submit the Proposal in accordance with the procedures outlined may be cause for disqualification. The Scope of Work for this project includes the evaluation of existing conditions, recommendation of repairs and preparation of engineering design drawings, technical specifications, and bid documents for construction of the Newport Reservoir Repair Project. The work associated with this project includes a facility evaluation and other tasks related to submitting recommendations for the repair and rehabilitation of the above facilities for seismic and safety concerns. BACKGROUND The Newport Avenue Reservoir, constructed in 1926, is located at Newport Avenue south of Lemon Hill Drive (Attachment 1). The reservoir is a partially buried, reinforced concrete structure with a wood framed corrugated metal roof. The reservoir is 100 feet in diameter with a nominal storage capacity of 1.4 million gallons (mg). A 1990 report prepared by Daniel Boyle Engineering (DBE) attached hereon recommended the installation of access ladders, railing, and sealing of wall cracks at this reservoir. There RFP: Newport Reservoir (CIP No. 6149) September 5, 2003 Page 2 of 8 are no as-built drawings available for this reservoir and this fact shall be addressed in the proposal. PROJECT GOALS The goals for this project are as follows: 2. 3. 4. 5. Evaluation of Existing Reservoir Alternative Analysis / Recommendations/Preliminary Cost Estimates General Administration, Meetings, and Presentations Provide Preliminary and Final Design Plans Complete design in a timely manner to meet construction deadline of June 30th, 2004. PROPOSAL FORMAT The Proposal must be concise, well organized and demonstrate the Consultant's and the individ.ual team memb'ers' qualifications and experience related to the proposed design. Discussion of relevant design experience should be included. The Proposal should be on one-sided 8½" by 11" paper, minimum arial 12 font. Include any preprinted material, past experiences, resumes, graphics, forms, etc. required to complete the Proposal package, which is to be bound or stapled. The Proposal package, including all documentation, shall not exceed ten 15 pages. The Proposal shall include, as a minimum, the following information: Executive Summary: A concise executive summary of the Proposal, identifying the entity/team being presented for consideration. The summary should include a brief history of the organization and background and relevant experience. The executive summary is limited to one page. Consultant Identification: Legal name of business, form of business (corporation, partnership, or other), location and address, telephone and fax numbers, e-mail address and contact person for response. Experience: Previous experience in the evaluation and design of reservoir, retrofitting, and improvement projects. Provide brief project descriptions, including contact information and dates of engagements. Key Staff: A list of key staff and their relevant qualifications that would be assigned to this project, including proposed sub-consultants and their role in this effort, if any. Include resumes of all key personnel listed. Approach and Scope of Work: Description of services to be provided and how the services will be accomplished. Include as one task the development of a Preliminary Design Report (PDR), which will details the evaluation process, retrofitting procedure, and make recommendations for reconstruction of the reservoir to meet the latest seismic and safety codes. Project Design Schedule: A detailed graphical project time schedL~le for completing the Project from Notice To Proceed through the completion of the plans, specifications, and construction cost estimate. A Preliminary Project Schedule is addressed in a subsequent section of this RFP. Consultant Services Agreement Acknowledgement: Include a brief statement acknowledging that the City's Consultant Services Agreement (CSA) is acceptable without modifications. If any section(s) of the CSA are not acceptable, Consultant is to indicate the section(s) that are not acceptable, the reason that the section(s) are not acceptable, and proposed revisions. Insurance and Business License Requirement Acknowledgement: Include a brief statement acknowledging that the City's insurance requirements and Certificate of Insurance are acceptable without modifications, and that the required business license(s) will be obtained. 10. Insurance Company Identification and "Best Rating" sheet: Consultant is to utilize the form in (Attachment 5) to identify the insurance company or companies that will be utilized to provide general liability, automobile liability, professional liability, workers compensation and employer's liability, and any other insurance necessary for the consultant's services relating to this project. Consultant is to indicate on the form the year the company was admitted in the State of California and its current rating based on the latest edition of Best's Key Rating Guide. The Project Manager, person authorized to sign on behalf of the Consultant, must sign and date the form. Please note that the City requires insurance companies to be rated A, VII or better, with the exception that Workers Compensation Insurance must have a rating of at least B, VII. Requirements for additional insured endorsements are also included on this sheet. 11. Project Cost: It is to be included and shall indicate the proposed "not-to- exceed" cost for completing the tasks identified and presented by the Consultant in the Scope of Work. The total "not-to-exceed" cost shall include the costs of all personnel, administration, overhead, meeting attendance, research, travel, postage, reproduction, and all other related costs necessary to complete the Project. Costs should be extended and summarized. PRELIMINARY SCOPE OF WORK The Consultant will work closely and in cooperation with the City during preparation of the design for the subject improvements. The selected Consultant is to provide the following services: 1. Coordinate development of PDR that provides alternative design studies, Design Plans, and Project Specification in accordance with City's Water Division. 2. Collect, utilize and coordinate all appropriate and available information for use in development of the design plan, including, but not limited to, the following: a) b_) c) d) City of Tustin Water Division information Site visit(s) accompanied by City's staff Any and all other pertinent reports and information Any and all other pertinent plan drawings Identify a point of contact and establish a communication plan with the City to ensure timely and accurate development of the design plan and specification. The Consultant is to develop the PDR, project plans, and specifications for bidding and construction of the new improvements. Submit plans and specifications to City's Building Division and incorporate comments as required. 6. The Consultant is to assist staff in bidding, submittal review, and construction support of project improvements. Prepare the Final Design Plan Set in reproducible format and provide 3 copies of the plan set and one electronic copy of the said design in CAD format and Adobe Final Design Plan Document files to the City. The Final Design Plan shall be in AutoCAD 2000 and Project Design Specification in Microsoft Word formats unless approved otherwise per the City of Tustin Mapping, Plan Preparation & Documentation Requirements (Attachment 2). S:\ClP Projects -Active\6149 - Newport Ave. Reservoir Repairs\Bid Process - Contract Approval\6149_RFP.doc RFP: Newport Reservoir (CIP No. 6149) September 5, 2003 Page 5 of 8 PRELIMINARY PROJECT SCHEDULE The following dates reflect the anticipated schedule for proposal solicitation and selecting the Consultant, awarding the contract, and completing the work requested in this RFP: · Solicit Proposals For Consulting Services 09/05/03 · Mandatory Pre Proposal Meeting at Project Site 09/22/03 · Proposals Due 09/29/03 · Interview/Negotiations 10/06/03 · Issue Notice of Award 10/10/03 · Sign and Approve Contract 12/01/03 · Completion of Preliminary Design 02/23/04 · Completion of Final Design 03/08/04 It is the responsibility of the Consultant to take into account the deadlines and other schedule requirements listed above as well as other relevant activities necessary for the completion of the required tasks. - CONSULTANT SELECTION The City may select for contract negotiations the firm that, in the City's judgment, will best meet the City's needs, irrespective of the comparison of fees and costs estimated by the proposers. The City's consultant evaluation and selection process is based on the following criteria. a. Understanding of the project requirements, including identification of critical elements and key issues. b. Technical approach and work plan for the project, including innovative approaches. c. Qualifications and experience of the Project Manager, Project Engineer, other key individuals, and sub-consultants. d. Results of reference checks. Reference checks will only be conducted for a short list of firms or the top rated firm. e. Clarity of the proposal. f. Compliance with proposal requirements. g. Level of effort in hours required completing the work, design schedule, and total fees in completion of the project. Selection of a firm to provide services will be based on demonstrated competence, professional qualifications necessary for the satisfactory performance of the services required and familiarity and prior experience with providing similar services. If a fee'can be negotiated that is deemed fair and reasonable to the City, the contract will be awarded to the selected firm. If a fee cannot be negotiated that is deemed fair and reasonable, we will thank the firm for the proposal and will proceed to the next most highly qualified firm to provide the requested services (with whom the City will seek to reach a negotiated fee and so on). CITY RESPONSIBILITY The City of Tustin shall provide Consultant with the following: Access to copies of all available existing data, information, reports, records and maps available in City files which may be relevant to this particular work. Access to the Newport Reservoir site. Access into the empty reservoir for inspection and documentation purposes. ~, project manager. _ Access to City Water Division staff and Building Officials Project title sheet will be furnished to the Consultant by KHR in hard copy to address applicable information and finalized by KHR. If Consultant provides claims made professional liability insurance, Consultant shall also agree in writing either (1) to purchase tail insurance in the amount required by the resulting agreement to cover claims made within five (5) years of completion of Consultant's services under the agreement, or (2) to maintain professional liability insurance coverage with the same carrier in the amount required by the resulting agreement five (5) years after completion of Consultant's services under this agreement. The Consultant shall also be required to provide evidence to City of the purchase of the required tail insurance or continuation of the professional liability policy by executing the attached letter agreement on Consultant's letterhead (included as part of Attachment 3). The City shall be named as additional insured and no policy may be modified or cancelled prior to thirty (30) days written notice. Attached for your information and use is a sample "Certificate of Insurance" (Attachment 4) utilized by the City of Tustin for such work. The Consultant should be aware that this is the City's standard document. No modifications or additions will be allowed to this document, and it is rbcommended that i~t be reviewed thoroughly prior to proposal submittal. City of Tustin Business License - As Consultant, it will be necessary to obtain a City business license at the expense of your firm prior to starting work. A City business license will also be required of any subcontractors your firm may use on this project. SUBMITTAL OF PROPOSALS If your firm is interested in submitting a letter proposal for this work, it must be received by 2 p.m. on September'~ 2003. Inspection, structural evaluation, and clean of the Newport Ave. reservoir. 1. Cleaning of reservoir - The objective is to clean the reservoir prior to the investigation of the reservoir. The cleaning will involve removing silt and debris from the interior of the reservoir by method of vacuuming during the dive investigation. This shall include bottom surfaces, columns, column bases, and appurtenances. The contractor shall be responsible for containing of silt for disposal, containment, and disposal. 2. Field evaluation of the reservoir utilizing Certified Divers/Engineers to visually inspect, photograph, and video visible deficiencies above and below the water line would entail site visit. a. Condition of appurtenances: overflow, inletJoutlet pipe, ladders, etc. b. Environmental and operating conditions which affect the use of the reservoir, maintenance of the reservoir, the aggressiveness of the atmosphere and water, and extent of abrasion damage. c. Inspection via inflatable raft greatly enhances the execution the reservoir roof structural survey. The City will fill-up the reservoir in providing an easy access for interior of reservoir from underline of roofline in conducting the visual inspection. d. Surfaces shall be photographed and entered into a Photographic survey album, which shall be included in the detailed written report. Where feasible, positive shut off of alt electrical current to reservoir for duration of inspection with a "Lock-Out, Tag-Out" system. 4. PROJECT UNDERSTANDING Schedule A for the Newport Reservoir involves a thorough evaluation of the 1.4 MG partially buried concrete reservoir regarding structural, condition, corrosion, safety and health issues. The evaluation will be accomplished by the diving method. In addition the reservoir will be cleaned to remove silt and debris from the interior surfaces of the reservoir. Based on the findings during the field evaluation, a preliminary design report would be prepared with observations, conclusions, recommendations and cost estimates to assist the City in determining the scope of work for the reservoir rehabilitation. The scope of work for Schedule A is typical of the work accomplished by HAE on a daily basis. The projects listed as experience are only a small sample of the projects that HAE has completed. APPROACH AND SCOPE OF WORK FOR SCHEDULE A Investigation and Preliminary Design Report 1. Kick-off meeting to verify approach to project 2. Accomplish field investigation of complete interior and exterior of the reservoir, to evaluate the present condition of the reservoir. a. Corrosion and structural soundness of the reservoir b. Condition of concrete regarding cracking and spalling and how it is protection the steel from corrosion, the percent failures, and their ability to be repaired. c. Condition of the expansion joints. d. Condition of the wood structural members and corrugated metal roofing materials. e. Environmental and operating conditions which affect the use of the reservoir, maintenance of the reservoir, the aggressiveness of the atmosphere and water, and extent of abrasion damage. f. Condition of appurtenances: float, overflow, inlet, ladder, etc. Field evaluation of the reservoir will be accomplished utilizing certified Divers/Engineers to visually inspect, photograph and video visible deficiencies. a. Evaluation of vapor zone is limited to visual from the water level below the highest point of the tank. With the water level 3 to 4 feet below roof line, inspection via inflatable raft greatly enhances the execution of the survey. Remove samples of interior sealant materials, coatings and exterior paints as needed to determine if they contain asbestos, lead, chromium compounds or zinc, as required by Title 22. A Preliminary Design Report (PDR) would be developed with photographic survey, observations, conclusions, and recommendations. a. Causes of deficiencies. The following sanitary measures will be fully complied with in accordance with AWWA C652-92, Section 5. 1. Diver's equipment is dedicated for use only in potable water storage tanks. a. No body parts shall come in contact with potable water. (1) Diver's suit shall be prophylactic in design. Only non- permeable dry-suits shall be used which incorporate an integrated hard helmet, neck and wrist dams, and boots. (2) Wet suits, hoods, and open or removable masks and face plates are not acceptable for use in potable water. b. Prior to diver entrance into potable water, diver shall be: (1) Fully dressed in prophylactic dry suit as detailed above. (2) All equipment and diver dress shall be thoroughly disinfected by a 100% wash-down with or immersion in a 200 PPM solution of chlorine and water. C. PROJECT UNDERSTANDING Schedule B for the Newport Reservoir involves providing as-built facility, site and topographic plan. This data will then be used by the Consultant to evaluate the reservoir and site regarding site drainage, site access, piping condition, appurtenances, tank circulation and safety issues. Based on the findings, a preliminary design report would be prepared with observations, conclusions, recommendations and cost estimates to assist the City in determining the scope of work for the reservoir rehabilitation project. To accomplish the scope of work for Schedule B, HAE would subconsult a portion of the work. Associated Engineers, Inc. will accomplish the surveying of the site to provide the as-built facility, site and topographic plan. In addition they will assist in the evaluation of the drainage and pipe condition. APPROACH AND SCOPE OF WORK FOR SCHEDULE B A. Investigation and Preliminary Design Report 1. Kick-off meeting to verify approach to project. Accomplish field surveying to prepare topographic survey and as- built site plan noting the following: Facility Boundary Existing ground conditions Drainage Patterns Piping, valves, vaults, equipment Reservoir appurtenances Accomplish field investigation to visually inspect and photograph the following conditions: On-site drainage and run-off Appurtenances, including reservoir circulation Access and site conditions in accordance with OSHA A Preliminary Design Report (PDR) would be developed with photographic survey, observations, conclusions and recommendations. Cause(s) of deficiencies Repair(s) including extent of deficient areas estimated costs, and life expectancy of repairs. Design improvements including extent of deficient areas, estimated design and construction costs, and feasibility of design in consideration of safety concern to adjacent properties and future replacement of the reservoir. 5. Jamev DFe, RCE--DirecWr of Utilities Newmark Groundwater Contamination Superfund Remedial Action, San Bernardino, California: Since 1995, Jamey Dye has been the acting project manager for the San Bemardino Municipal Water District on the Newmark Groundwater Superfund Remedial Action. As project manager Jamey has designed and overseen the construction of seven production wells ranging in depth from 500 feet to 2000 feet, 50,000 of water lines, treatment plants totaling 16.5 Million Gallons Per Day, and a booster stations. This project including DHS permitting, public relations, Coordination with the USEPA, basin management, and basin modeling. Enhanced Reliability Schedule of Improvements, San Bernardino, California.' As Senior Engineer of the City of San Bernardino Water Department, Jamey Dye was on a design development team for Enhanced Reliability Schedule of Improvements (ERSI). The ERSI is a $50M five phase capital improvement project including 24 Million Gallons of above ground storage, 40,000 feet of 16" through 60" pipelines, booster stations, pressure zone realignments, and wells. Jamey Dye, RCE Utility Director Jamey Dye has more than 14 years of experience in civil engineering, project management, and construction contract administration. He has experience in design, and construction management for residential, commercial and industrial subdivisions, water wells, booster stations, water treatment plants, and over 50,000 feet of pipeline up through 36" diameter. He has worked as a project manager in both the public and private section and is a State of California Certified Water Treatment and Distribution Operator as well as a Registered Civil Engineer. Water Master Plan for the City of San Bernardino Municipal Water Department - As senior engineer, Mr. Dye was on a team to Update the SBMWD 1990 master plan. This effort began in 2002 and turned into a completely new master plan including land use analysis, hydraulic modeling, efficiency modeling, finance planning, water supply, water treatment, and distribution master planning. Supervising and Engineering Section of the Water Department: As supervisor of the Engineering Section of the City of San Bemardino Water Municipal Department, Jamey was involved in all aspects of water utility management, his responsibilities included backflow prevention, well maintenance, drilling, energy optimization, water quality, water discharge, emergency response, State drinking water Permits, booster station design, development of standard drawings, and pipeline design. Enhanced Reliability Schedule of Improvements: Jamey was a key member of a team of department staff and outside consultants who developed the first phase of a long-term master plan for the water system. This involved bringing together a large amount of information and developing a plan that included reservoirs, boosters, and pipelines to support long-term growth for the City of San Bernardino through 2055. This project included applying for and receiving funds. Former Norton Air Force Base Water System Upgrade: Jamey was responsible for the upgrade of the water system at the former Norton Air Force Base including, evaluating the existing water system, developing a proposed plan, completing grant applications, designing the pipelines and wells, and managing the construction. Newmark Groundwater Contamination Superfund Remedial Action: As a Project Manager for the City of San Bernardino Water Department, Jamey was responsible for the design and construction management for four water treatment plants with a combined capacity of 40 Million Gallons, 30,000 feet of water main, 10 production wells, 30 monitoring wells, and associated water facilities. Upper Santa Ana Water Resources Association: During James's tenure at the City of San Bernardino Municipal Water Department, he was very involved in regional water management issues. Mr. lsnasious designed the backbone reclaimed water pipeline at San Elijo, San Diego County to provide water to San Alijo Gold Course.
