La Palma Ave, Anaheim, CA 92806 | (800) 400‐9353 | www.ATIrestoration.com ATI Scope of Work – Addendum 2 73797568;2 Page 2 of 5 Date: 01-17-2024 To: City of Tustin Tustin Hangar Fire Incident 300 Centennial Way Tustin, CA. 92780 From: Tim Nunn Jerry Cielak Executive Project Director Executive Vice President ATI Restoration ATI Restoration Re: Tustin Hangar Fire Incident – Tustin Youth Center Based on the direction from the City of Tustin, Incident Management Team (IMT) and the IMT's consultant, IEM, we provide this Scope of Completed Work (the “Additional Work”), and pursuant to the terms of the Consulting Agreement and initial Scope of Completed Work for the subject project, provide this addendum and supplemental scope of work. All of the terms and conditions of the Consultant Services Agreement between the City and ATI continue to remain in place and apply to this Additional Work. General Items: 1. ATI shall possess and furnish all necessary equipment, materials, and labor to adequately perform the specified services in this Addendum 2 scope letter. 2. ATI shall maintain a copy of all current MSDS documentation and safety certifications at the site at all times, as well as comply with all other site documentation requirements of applicable OSHA, SCAQMD, EPA programs and this specification. Scope of Work: The following material is presented in an order that ensures all affected areas are properly addressed from a procedural point of view. The sequence of events, in which these procedures will be performed, is discussed later in this Scope of Work under Critical Path Management Program (CPM). • ATI will provide any and all resources, consumables and equipment to perform the work as directed by the Incident Action Plan (IAP). • ATI will require all employees to sign in/out logs before entering or exiting the work areas. • ATI will hold and log On-site Safety meetings on a daily basis • ATI employees will be properly supervised with a management to technician of: ATI HEADQUARTERS 3360 E. Due to the nature of the event and the specifics related to the damages this list is not guaranteed to be all inclusive.  Executives  Project Directors  Operations Managers  Project Managers  Resource and Logistics Coordinators  Health, & Safety Specialists  Project Accountants  Asbestos Abatement Supervisors  Asbestos Abatement Technicians  Third Party Consultants Please note that ATI will scale up or scale down as requested by customer’s priorities. • ATI will work 7 days per week until otherwise directed. • ATI will work with IMT, IEM or their assigned representatives to address any special needs that need to be prioritized. • ATI will provide a certificate of insurance with the client’s named as additional insured. ==================================================================== ATI Scope of Completed Work: • Work was completed on November 30, 2023. • Mobilization of resources, equipment, materials, supplies and trailers. • All personnel in the work area shall be outfitted in Tyvek style protective clothing and dual cartridge HEPA filtered negative air half faced respirators. • Provide showers, toilets and wash stations for workers. • WHEREAS, City has declared an emergency under the Emergency Services Act, Government Code §§8500-8655 (the “Act”) and requires the emergency services of an abatement consultant to provide asbestos abatement and related services (“Abatement”) related to the release of asbestos from the North Hangar, including Abatement at: 1. HEPA vacuum and/or wet wipe all horizontal surface including but not limited to bathroom partitions, sinks, counter tops, toilets, urinals, and all interior flooring. 2. All areas that previously that tested ab ove thre sholds inclu ding the Pre school So uth Entry floor, Kitchen countertops, and the cl assroom floor we re wet wiped and heap vacuumed per AQMD Rule 1403. 3. Remove visible potential asbestos debris from fields, bathroom facilities, parking lots, children play areas, walkways, planters, and any accessible areas. 4. After visual inspection was completed by a third party hygienist ATI personal replaced and reinstalled all removed items. ATI HEADQUARTERS 3360 E. La Palma Ave, Anaheim, CA 92806 | (800) 400‐9353 | www.ATIrestoration.com ATI Scope of Work – Addendum 2 73797568;2 Page 4 of 5 Clarifications • ATI will provide all in -house equipment (ladders - hand tools) and building materials to perform our scope of work. • Prevailing Wages are to be paid to all onsite personnel. Exclusions - The following is specifically excluded from our work: • ATI has not included any bid or performance bonds in this proposal, any gas, electric or water failure is excluded. ===================================================================== CRITICAL PATH MANAGEMENT Understanding the sense of urgency inherent in projects of this nature, a flexible approach to the restoration project is mandatory. Following acceptance of this Scope of Work, a Critical Path Management (CPM) Program will be established that will outline the definitive sequence of events and their corresponding time frames for completion of each event. This CPM Program will be formulated based solely on the sense of urgency as reflected by the client, considering each phase of this operation. Coordination of all phases of this restoration project is critical to the successful, timely, and cost-effective completion of the Work. The sequence in which the Work will be performed will be discussed following determination of the Critical Path. IMPORTANT POINTS Some cleaning procedures outlined above may be changed at the discretion of the IMT or IEM to maximize effectiveness and efficiency. Any changes or alterations to this Scope of Work at the request of the IMT or IEM, may cause an adjustment to the total project cost. It is assumed that ATI will have necessary access to the facility. Due to the unknowns in dealing with losses of this nature, some methods of the cleaning effort may require adjustment as the job progresses. Daily communication is critical for the success of any project. In an effort to keep all interested parties apprised of the status of this project, we request that the ATI’s on-site Project Manager and a designated representative from the client meet daily. It is preferable that this representative have decision -making authority regarding any changes, either additions or deletions, to this Scope of Work. ATI HEADQUARTERS 3360 E.
